This simple iStation data tracker is awesome because kids can handle it so easily. You have two choices on the bottom tab: August to May and September to June. 

Each month, kids use their test results to plot and monitor growth with a crayon, colored pencil or marker. Since it's editable, just make a copy when prompted and you can edit the raw scores along the Y axis as needed.
